    CHIEF NURSING OFFICER

    Under the direction of the President and Chief Executive Officer, provides leadership, direction, and administration for the departments of Nursing; ensures compliance with established objectives and the provision of quality health care services; acts in collaboration with the President and Chief Executive Officer; and assists the Chief Executive Officer with other administrative functions as assigned. The Chief Nursing Officer is responsible for the direction of the hospital’s nursing services and participates as a member of the executive management team, sharing authority and responsibility with other executive managers to assure the provision of quality, cost efficient healthcare to patients presenting for services. The Chief Nursing Officer has the authority, accountability and responsibility to establish nursing policies and procedure, nursing standards of patient care, treatment, and services, and standards of nursing practice and nurse staffing plans for the organization. 

     

    RESPONSIBILITIES:

     

    • Establishes and monitors the quality of nursing care, treatment and services delivered to patients throughout the facility. 
    • Coordinates development of hospital wide patient care programs, policies and procedures that describe how patients’ nursing care needs, or the needs of patient populations receiving nursing care, treatment, and services are assessed, evaluated and met. 
    • Develops and implements the hospital’s plan for providing nursing care, treatment and services to those patients requiring nursing care, treatment and services. 
    • Participates with the governing body, management, medical staff and clinical leaders in the hospital’s decision-making structures and processes. 
    • Implements an effective, ongoing program to measure, assess, and improve the quality of nursing care, treatment and services delivered to patients. 
    • Communicates and interprets Gardens Regional Hospital’s philosophy of nursing, plan for nursing care and Patient Care services goals and objectives to the nursing staff, administration, the medical staff and other departments within the organization. 
    • Establishes a strategic and conceptual framework for patient care management within the facility, taking into account patient mix, community needs, staff skills and knowledge, available personnel and material resources and healthcare expectations. 
    • Works collaboratively with others to create and execute integrated business plans.  Identifies short and long range goals as well as desired outcome measures and develops a plan of implementation, monitoring, reporting and evaluating. 
    • Establishes nursing policies and procedures, nursing standards of patient care, and standards of nursing practice and nurse staffing plans. 
    • Oversees the development and implementation of policies and procedures that guide and support the provision of patient care consistent with professional organizations’ and regulatory bodies’ standards, organizational policy, goals and strategic initiatives. 
    • Approves nursing policies and procedures, nursing standards of care, treatment, and services, and standards of nursing practice before implementation. 
    • Ensures that the patient care units and services are in compliance with Accrediting Agencies, Federal and State regulations and professional organization standards and that quality control programs are maintained as appropriate. 
    • Oversees the development and monitoring of departmental budget and all related expenditures including recommendations of a sufficient number of qualified and competent staff, space and other resource needs ensuring maximization of resources, efficiency and cost-effectiveness of patient care operations. 
    • Promotes the collaboration of nursing, other clinical disciplines, the medical staff and the administrative staff in the development and implementation of performance improvement activities to improve patient care. 
    • Oversees the coordination of ongoing educational offerings for nursing; assures regular needs assessments and program planning to address these needs; monitors environmental factors affecting education/development of staff and defines resources available to address same.
    •  Ensures consistent application of Human Resource policies and procedures; collaborates with Human Resource Department to recruit, select and retain qualified personnel.

      

    EDUCATION/TRAINING/EXPERIENCE:

    • Master’s degree in Nursing
    • BCLS / ACLS
    • Five (5) years nursing management experience at Chief Nursing Officer level (or next lower level) in an acute healthcare facility.
    • Demonstrated management, administrative, scheduling, planning and budgeting skills.
    • Working knowledge of state, federal and accrediting agency (DNV, TJC or AOA) nursing regulations / requirements governing aspects of nursing including but not limited to Title 22 and CA Nurse Practice Act
    • Bilingual (English/Spanish) preferred
    • Excellent communication (written / verbal) skills

    CLINICAL DOCUMENT IMPROVEMENT (CDI) SPECIALIST

    The CDI SPECIALIST is responsible for reviewing medical records to facilitate the accurate representation of the severity of illness by improving the quality of the physicians’ clinical documentation.  This position facilitates improvement of clinical documentation in the medical record to ensure an accurate level of clinical services, appropriate coding of principal and secondary diagnoses, complications and POA indicators that drive reimbursement, quality performance metrics and CMI.  Activities involve extensive record review as well as interaction with physicians, HIM and Nursing staff.

     

    PRIMARY RESPONSIBILITIES:

     

    • Assists in the development of both formal and informal education for physicians, nursing staff and other clinical staff.
    • In collaboration with physician leadership, designs and implements specific tools to support medical record physician documentation.
    • Identifies strategies for sustained work process changes that facilitate complete, accurate clinical documentation.
    • Reviews inpatient Medical Record for identified payor populations on admission and throughout hospitalization (concurrent review).   Analyzes clinical status of patient, current treatment plan and past medical history and identifies potential gaps in physician documentation.  Submits queries to medical staff for clarification of documentation.
    • Works collaboratively with coding staff to assure documentation of discharge diagnoses and any co-existing/co-morbidities are a complete reflection of the patient’s clinical status and care.
    • Improves the overall quality and completeness of clinical documentation in the medical record in accordance with all regulatory requirements.

     

    EDUCATION/TRAINING/EXPERIENCE:

    • Bachelor’s degree with course work in human anatomy and/or physiology, medical terminology and disease process required.
    • Graduate of a program of Nursing (BSN), Health Information Management (RHIT/RHIA) or foreign medical doctorate degree strongly preferred.
    • Minimum 2 years in an acute care setting with minimum of 6 months performing clinical documentation improvement (CDI) functions.
    • Certified Coding Specialist (CCS), Certified Coding Associate (CCA) or Certified Documentation Improvement Practitioner (CDIP) certification preferred.
    • Certified Clinical Documentation Specialist (CCDS) credential with recent experience preferred.
    • Coding skills with experience in ICD-9/10 CM and working knowledge of AHA Coding preferred.  ICD-10 certification preferred.
    • Basic Computer skills – familiar with windows based software programs.
    • Critical thinking, problem solving and deductive reasoning skills.
    • Effective communication skills (verbal and written).
    • Excellent organizational, analytical and interpersonal skills.
    • Extensive knowledge of Medicare Parts A and B and their impact on DRG assignments.

    SOCIAL WORKER / DISCHARGE PLANNER

    The Social Worker is responsible for assessing patient and family psycho-social needs and discharge planning issues related to medical treatment.   This position will provide crisis intervention, emotional support, community resource information, discharge planning and legal reporting.  The Social Worker is responsible for helping patients and families cope with problems they are facing to improve the patient’s life.   The Social Worker will identify and evaluate patient need for social services, specifically relating to acceptance of illness, debility and/or medical recommendations, adjustment of medical setting, and personal or situational issues with patient and family members.   In addition, the Social Worker will assess and plan for discharge needs and identify and facilitate provision of services necessary to meet discharge planning needs facilitating a safe and timely discharge.

     RESPONSIBILITIES:

    • Immediate review of patient cases as identified by hospital policy.  Completes care plan as identified during initial admission assessment.
    • Investigates referrals of all types of abuse and reports to regulatory agencies as required
    • Attends to high risk patients
    • Prepares written psycho-social evaluation/assessment and comprehensive statements
    • Consults with medical staff to ensure a treatment plan that meets the psycho-social needs of the patient
    • Participates in Interdisciplinary Team meetings
    • Prepares written discharge planning assessment and documents in patient medical record
    • Assist patient and family in locating alternate level of care facility that meets patient and family needs
    • Assists Case Managers with difficult patient placement issues
    • Provides information and referrals to local agencies and/or hospital services
    • Discusses patient needs and available community resources with patient, family and attending physician.   This includes alternate level of care such as acute rehabilitation, skilled nursing facility or extended care as well as any need for durable medical equipment (DME)
    • Participates in case management meetings to present, discuss and/or coordinate patient discharge planning
    • Prepares and presents appropriate UR/QA/PI information

    EDUCATION/TRAINING/EXPERIENCE:

    • Bachelor’s Degree in Social Work or related field – MSW or LCSW preferred
    • BCLS
    • One (1) year experience in an acute care facility
    • Demonstrated social work assessment and counseling skills
    • Knowledge of state and federal programs that provide medical care and financial support to individuals

    The Medical Staff Coordinator provides assistance to the Medical Staff Director.  The Medical Staff Coordinator is a liaison between the Director and the medical staff.  This position is knowledgeable of credentialing functions, proctoring guidelines and on-going performance monitoring.  Candidate must possess a working knowledge of medical staff bylaws, rules and regulations and all state and federal regulatory requirements and accreditation standards.

    • Works to develop standardization of products and equipment throughout the hospital.

    EDUCATION/TRAINING/EXPERIENCE:

    • Bachelor’s degree or equivalent experience in acute care Materials Management
