    CHARITY CARE COORDINATOR

    The Charity Care Coordinator is responsible for facilitating, resolving and responding to eligible patients in need of financial assistance in a positive, professional and empathetic manner in accordance with GRHMC Charity Care Policy and in accordance with certain State and Federal charity and term payment programs offer by GRHMC – either in conjunction with or separate from any external vendors GRHMC may use.  Work with patients to process all charity care applications within the time frames stated in established policies and procedures.   Position also handles all estate filings/deceased accounts and bankruptcy cases.

      

    RESPONSIBILITIES:

     

    • Assures compliance with GRHMC, State and Federal charity or payment plan programs participated in by GRHMC.    Assures timely adjustments for charity and payment plan programs.
    • Reviews patient’s insurance information, payments, contractual adjustments, account history and employment status determining charity care or bad debt eligibility.
    • Reviews Charity Care Application for completeness, accuracy and required supporting documentation.  Using medical record, confirms demographic information.
    • Schedule incomplete applications for follow-up.
    • Conducts asset search for unreported property.
    • Confirms FPIG eligibility.
    • Prepares appropriate forms (Charity Care Approval Write-Off, Contractual Adjustments, Financial Class Change, etc.)
    • Prepare monthly charity care log.
    • Review accounts identified as possible Bad Debt.  Assign as bad debt and assign to appropriate vendor.  When appropriate, determine medical insurance eligibility and provide appropriate information to GRHMC billers.
    • Review appropriate vendor invoices. Perform variance analysis of vendor commissions, submit findings report and request refund, if appropriate.
    • Reviews bankruptcy notices and takes appropriate action on accounts.
    • Promotes public image of Gardens Regional Hospital and Medical Center.
    • Provides timely and effective decisions by good analysis of data.
    • Assures all telephone calls are handled properly and courteously.
    • Maintains the privacy and security of all confidential and protected health information.   Uses and discloses only that information which is necessary to perform the function of the job.
    • Performs related duties as assigned.

     

    EDUCATION/TRAINING/EXPERIENCE:

    • High school graduate or equivalent, Bachelor’s degree in Business Administration or a Health Care related field preferred
    • Two (2) years’ experience in hospital/medical billing and/or collections
    • Two years’ Fee for Service billing operations expreience
    • Bilingual (English/Spanish) preferred
    • Excellent communication (written / verbal) skills
    • Excellent organizational skills

    SOCIAL WORKER / DISCHARGE PLANNER

    The Social Worker is responsible for assessing patient and family psycho-social needs and discharge planning issues related to medical treatment.   This position will provide crisis intervention, emotional support, community resource information, discharge planning and legal reporting.  The Social Worker is responsible for helping patients and families cope with problems they are facing to improve the patient’s life.   The Social Worker will identify and evaluate patient need for social services, specifically relating to acceptance of illness, debility and/or medical recommendations, adjustment of medical setting, and personal or situational issues with patient and family members.   In addition, the Social Worker will assess and plan for discharge needs and identify and facilitate provision of services necessary to meet discharge planning needs facilitating a safe and timely discharge.

     RESPONSIBILITIES:

    • Immediate review of patient cases as identified by hospital policy.  Completes care plan as identified during initial admission assessment.
    • Investigates referrals of all types of abuse and reports to regulatory agencies as required
    • Attends to high risk patients
    • Prepares written psycho-social evaluation/assessment and comprehensive statements
    • Consults with medical staff to ensure a treatment plan that meets the psycho-social needs of the patient
    • Participates in Interdisciplinary Team meetings
    • Prepares written discharge planning assessment and documents in patient medical record
    • Assist patient and family in locating alternate level of care facility that meets patient and family needs
    • Assists Case Managers with difficult patient placement issues
    • Provides information and referrals to local agencies and/or hospital services
    • Discusses patient needs and available community resources with patient, family and attending physician.   This includes alternate level of care such as acute rehabilitation, skilled nursing facility or extended care as well as any need for durable medical equipment (DME)
    • Participates in case management meetings to present, discuss and/or coordinate patient discharge planning
    • Prepares and presents appropriate UR/QA/PI information

    EDUCATION/TRAINING/EXPERIENCE:

    • Bachelor’s Degree in Social Work or related field – MSW or LCSW preferred
    • BCLS
    • One (1) year experience in an acute care facility
    • Demonstrated social work assessment and counseling skills
    • Knowledge of state and federal programs that provide medical care and financial support to individuals
    • Bilingual (English/Spanish) preferred
    • Excellent communication (written / verbal) skills
    • Excellent organizational skills
